Yes cocaine is an unquestionably worse drug in term of harm to the user, harm to society, and potential for abuse and addiction.
The most recent studies have actually shown ecstasy (MDMA) to be one of the safest drugs of abuse, even safer than marijuana. The number of deaths due to MDMA is very small, unfortunately most deaths are a result of contaminants and “filler” drugs in ecstasy tablets. The vast majority of tablets are NOT pure and most death is due to one or more of the other substances in the tablet. Aside from that there have been some deaths from mixing MDMA with prescription drugs, especially MAOI’s (a class of antidepressants). Heavy drinking on MDMA also substantially increases risks. People who use MDMA typically become dehydrated because of the drug, because of dancing for a long time, forgetting about drinking, and so on and this is greatly worsened by alcohol, a diuretic.
The only cause of death from pure MDMA aside from interactions with other drugs is either dehydration or hyponatraemia. Polydipsia is a condition where a person has excessive thirst and may continue to drink water which can lead to water intoxication and hyponatraemia, a very dangerous electrolyte disturbance indicating low concentrations of salt in blood plasma. However the likely hood of this occurring is small and with medical intervention treatment can be provided.
Cocaine is literally the worst thing for the heart, not only is it a powerful stimulant it is also a local anaesthetic and as a local anaesthetic cocaine blocks sodium channels, altering action potentials (the nerves ability to fire- sending messages). By acting on sodium channels and increasing blood pressure and heart rate risk of cardiovascular damage is significant and risk of myocardial infarction (heart attack) is significant after using and chronic use greatly increases risk of cardiovascular complications and death. Cocaine is also highly addictive (for more so than MDMA), studies with animals have shown that when a cocaine addicted animal is given unlimited access to the drug it will ignore all other things (eating, drinking) and will use until death. Not even heroin has the addictive potential as cocaine.
